Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers and MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, or related public sector field.
• 2+ years in family systems, case management, or related areas.
• 2+ years with Ohio delivery systems and regional knowledge.
• Must reside in the West Region of Ohio.

Responsibilities

• Coordinate care plans and monitor progress to meet member needs.
• Evaluate referrals for eligibility and propose an appropriate care approach.
• Collaborate with care managers and Medical Directors to optimize outcomes.
• Document care processes per regulatory and accreditation guidelines.
• Assist members in remediating care gaps and coordinating services.
• Travel within Western Ohio region up to 50% of the time.
