Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years using MS Office (Outlook, Word, Excel, SharePoint).
• 2+ years in child and family services or related public sector health care.
• Experience with Ohio delivery systems and local networks.
• Willing to travel within West Region Ohio up to 50%.

Responsibilities

• Collaborate with care management teams to coordinate services for members.
• Evaluate member needs, eligibility, and develop care plans.
• Monitor progress and document care activities in compliance with guidelines.
• Coordinate with care managers, supervisors, and medical directors to optimize outcomes.
