Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, or a related public sector field.
• 2+ years experience in family systems, case management, or child development.
• 2+ years experience with Ohio delivery systems.

Responsibilities

• Coordinate and manage care plans for members in the OhioRISE program.
• Travel within Western Ohio up to 50% and coordinate with member homes.
• Collaborate with Child and Family Teams and other case managers.
• Document care management activities in compliance with guidelines.
