Job summary

CVS Health Corporation is seeking a Case Management Coordinator for the West Region of Ohio. The role is full-time and field-based with significant travel to member homes within Western Ohio.

Responsibilities include care coordination, evaluating member needs, and facilitating care plans in alignment with OhioRISE Medicaid goals. A Bachelor's or Master's in behavioral health is required, with strong expertise in child and family services and 2+ years in related settings.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, or related public sector field.
• 2+ years in family systems, case management, counseling/therapy, child protection, or child development.
• 2+ years experience with Ohio delivery systems and local networks.
• Must reside in the West Region of Ohio ( Butler, Hamilton, Clermont, Warren, Montgomery, Allen, Hardin, Defiance, or Lucas).
• Willing and able to travel up to 50% of the time; some trainings may require travel to New Albany office.

Responsibilities

• Collaborate with case management team to coordinate and inform care plans.
• Evaluate member needs, eligibility, and appropriate care approaches.
• Monitor care plan progress and adjust as needed.
• Consult with Medical Directors and other programs to overcome barriers to goals.
• Advise and empower members to participate in healthcare decisions.
• Serve as a single point of contact to remediate gaps in care and access.
• Document care using established case management processes.
