CVS Health is recruiting a Case Management Coordinator in Ohio to support the OhioRISE program. This full-time role requires travel within Northeast Ohio to member homes and other locations, up to 50% or more.

Strong coordination and communication are essential to improve outcomes for children and families. The position emphasizes case management, care coordination, and collaboration with multidisciplinary teams.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years in behavioral health, social services, or human services.
• Experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, or related field.
• 2+ years in family systems, case management, or related areas.
• 2+ years with Ohio delivery systems and local networks.
• Willing to travel up to 50% of the time; some travel to the New Albany office may be required.
• Reliable transportation; mileage reimbursed.

Responsibilities

• Be clinically and culturally competent with training to manage complex cases.
• Evaluate members' needs and plan care accordingly.
• Coordinate and implement care plan activities and monitor progress.
• Collaborate with Medical Directors and health programs to improve outcomes.
• Work with Child and Family Teams and escalate quality issues as needed.
• Use negotiation and motivational interviewing to engage members
• Provide coaching and empower members to make informed health decisions.
• Serve as single point of contact and remediate gaps in care.
• Monitor, evaluate, and document care per policies and regulations.
