Qualifications

• Must have 2+ years in behavioral health, social/human services.
• Experience with children’s mental health or public sector human services is required.
• Proficient in MS Office Suite (Outlook, Word, Excel, SharePoint).
• Able to travel within Western Ohio up to 50% of the time.

Responsibilities

• Conduct comprehensive evaluation of referred members’ needs and eligibility.
• Coordinate and implement care plan activities; monitor progress.
• Collaborate with case managers and health programs to overcome barriers to goals.
