Job summary

Rise Services, Inc. is hiring for a Family Support Specialist in the Phoenix, AZ area. The role is remote with occasional in-office work from the Surprise, AZ office, delivering family connections and nurturing parenting programs with in-home case management and supportive services to families.

You will coordinate services, meet with families, and support positive outcomes. A Bachelor’s degree is required, along with strong communication skills, cultural sensitivity, and the ability to travel.

Qualifications

• Bachelor’s level degree required with related work experience with families.
• Strong verbal and written communication skills; able to communicate with multiple stakeholders.
• Willingness to work flexible hours including evenings; travel as needed.

Responsibilities

• Provide short term, time-limited coordination of services and resources to families.
• Meet with families regularly to address and resolve issues using evidence-based curriculum and community resources.
• Support families with case management and collaboration with the Team Lead.
• Ensure contract timelines with weekly progress notes and tasks.
• Be flexible to meet families' schedules; travel around county or metro area is required.
