Job summary

InnovAge Holding Corp. seeks a Remote Phone Triage Registered Nurse to provide after-hours nursing triage by phone for participants and caregivers. You will work with the PACE center staff and providers to guide care and document assessments in the EMR.

The role is 100% remote with a Monday–Friday shift from 3:00 PM to 11:00 PM MST. A strong nursing background, geriatric care focus and reliable internet are required.

Qualifications

• Associates degree in nursing required.
• Current California and Compact RN license required.
• High-speed Internet access required.

Responsibilities

• Triage participant, caregiver and facility calls to determine priority and route as needed.
• Document all clinical interactions in EMR per InnovAge standards.
• Provide health teaching to participants and families.
• Coordinate handoffs between shifts and centers to ensure continuity of care.
• Assess patients against standards of practice and develop comprehensive care plans.
• Identify variances and contribute to quality improvement discussions.
