Job summary

InnovAge is seeking a Remote Phone Triage RN to provide after-hours nursing triage by phone for participants supported by PACE. The role operates 3:00 PM–11:00 PM MST, Monday to Friday, and collaborates with clinic teams to deliver high-quality assistance.

The ideal candidate holds an Associate degree in nursing with a California license (and Compact eligibility), and demonstrates strong assessment and documentation skills for remote care delivery.

Qualifications

• Active California RN license and valid Compact license.
• Associate degree in nursing is required.
• High-speed internet access for remote work.
• Bi-lingual abilities are preferred.

Responsibilities

• Triage calls to assess priority of care and route accordingly.
• Accurately document clinical notes in EMR and route orders as needed.
• Provide health teaching to participants and families on various conditions.
• Coordinate handoffs between shifts and after-hours care with staff.
• Collaborate with clinic team to ensure patient-centered, proactive care.
