InnovAge is seeking an Visiting After Hours Registered Nurse (RN) to join the comfort care and after-hours team. This salaried, remote role involves on-call triage, potential home visits, and coordination with the IDT and on-call physician.

Responsibilities include assessing needs, providing nursing treatments, and educating participants and families while maintaining high standards of care across the PACE program.

Formación

• Current State issued Registered Nurse license.
• Current First Aid and BLS certifications required prior to hire.
• 2+ yrs health care with geriatrics, at least 1 year home health.
• Reliable personal transportation, valid driver's license, good driving record and auto insurance.

Responsabilidades

• Respond to after-hours calls and triage care, including home visits as needed.
• Coordinate with IDT and on-call provider for appropriate interventions.
• Document care in EMR per InnovAge standards.
• Educate participants and families on end-of-life care and comfort measures.
• Ensure a safe patient environment and compliance with policies.
