TRU Community Care is seeking a Hospice Phone Triage RN to join our afterhours triage team, taking calls from patients and collaborating with visit nurses to determine the best clinical course. The role supports triage protocols and may direct patients to emergency care or inhome visits as needed.

This position remote after training, requires residency in CO, FL, or GA, and an active CO or compact RN license plus one year of RN experience (hospice, home health or PACE preferred).

Qualifications

• Must possess a current RN license in the state of Colorado or compact nursing license.
• Must reside in Colorado, Florida, or Georgia.
• Must have a minimum of one year RN experience, with hospice, home health, or PACE experience preferred.
• Must successfully pass agency required background checks and drug screening.
• Must have secure HIPAA-compliant workspace with reliable wired internet.

Responsabilités

• The Triage Registered Nurse performs in a supportive role to the Care Team by answering calls coming into the agency, with response to the telephone call by delegating tasks.
• Monitors and responds to triage calls within the agency specific time limits.
• Evaluates and assesses patients’ health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines.
• Obtains appropriate input and direction from the provider as needed to determine the best clinical course of action and recommendations for specific patient health care needs.
• May direct them to emergency room, urgent care, provide home care advice, or arrange for an in-home nursing visit.
• Utilize triage protocols appropriately.

Formation

RN license in Colorado or compact nursing license

Residency in Colorado, Florida, or Georgia

Minimum 1 year RN experience (hospice, home health or PACE preferred)
