Job summary

Mercy Health seeks an RN Triage Specialist to provide telephonic triage and virtual monitoring, guiding callers to appropriate care using established workflows and protocols. You will document in the EMR, participate in care coordination, and support remote or on-site operations as needed.

The role emphasizes safety, quality, and patient experience, with evenings, weekend rotations, and holiday coverage. Requires RN licensure and ADN/BSN considerations.

Qualifications

• RN license required in applicable state(s).
• Multistate/Compact RN Licensure preferred.
• ADN or Diploma Nursing required.
• BSN preferred.

Responsibilities

• Provides telephonic triage or requested support and / or virtual monitoring. Offers subsequent recommendations, education or care advice using decision making tools, clinical judgement, and defined workflows.
• Participates in care coordination, by partnering with customers to reduce readmissions, enhance chronic disease management, manage health risk and injury reporting. Schedules provider appointments and facilitates provider communication.
• Ensures accurate, timely documentation in the EMR according to best practice, guidelines, or workflows.
• Participates in virtual monitoring and subsequent reporting and escalation to support services identified to ensure patient needs are met and continuity of operations is maintained.
• Participates in process improvement, professional development, peer development and peer review
