Isaac Health is seeking a compassionate Patient Care Coordinator to serve as the first point of contact, blending a patient-first approach with proactive engagement. You will conduct initial consultations, perform assessments, and communicate the value of medical consultations, therapy sessions, and cognitive assessments.

You will orchestrate seamless care journeys, coordinating sessions among patients, caregivers, and medical teams.

Qualifications

• Excellent verbal and written communication for patients, medical team and staff.
• Fluent in Spanish.
• Professional, patient, and caring attitude with patients and families.
• Demonstrated call center experience with high-volume calls.
• Telehealth experience is a plus.
• Familiarity with Medicare and Insurance concepts.
• Accurate and complete patient documentation and typing skills.
• Able to navigate multiple systems while documenting during calls.
• Strong tech-savviness and quick learning of new software systems.
• Proficient with Google Office Suite.

Responsibilities

• Reach out to patients interested in care to explain Isaac Health’s services.
• Coordinate cognitive health screening campaigns.
• Take medical history from patients or caregivers.
• Schedule and coordinate appointments.
• Assist patients with technology and questions.
