Job summary

Isaac Health is seeking a mission-driven coordinator to be the first contact, guiding patients through consultations, therapy, and cognitive assessments. You will coordinate care journeys, communicate value, and work with the medical team, caregivers, and external partners to ensure seamless patient experiences.

The role emphasizes remote work, strong communication, empathy, and the ability to learn new systems while maintaining accurate documentation and adherence to workflows.

Qualifications

• Excellent verbal and written communication with patients, medical team and internal staff.
• High professionalism, patient care, and empathy towards patients and families.
• Demonstrated call center experience handling high-volume calls.
• Prior telehealth experience is a plus.
• Familiarity with Medicare and Insurance concepts.
• Accurate patient documentation and multi-system data entry.

Responsibilities

• Reach out to patients who expressed interest in care and explain services.
• Coordinate cognitive health screening campaigns.
• Take medical history from patients or caregivers.
• Schedule and coordinate appointments.
• Assist patients with technology and other questions.
