Qualifications

• 2-3 years of healthcare operations or care coordination experience.
• Medical records or hospice compliance experience.
• Proficiency with HCHB and related agency systems.

Responsibilities

• Coordinate daily clinician schedules and staffing assignments.
• Serve as the primary point of contact for scheduling and staffing communications.
• Collaborate with Intake to confirm staffing availability for new admissions and referrals.
• Process scheduling adjustments, reassigned caseloads, missed visits, and visit updates.
• Monitor staff coverage and communicate gaps to leadership.
• Maintain records of employee availability, competencies, and productivity metrics.
• Maintain patient records and ensure compliance with hospice regulations.
