Job summary

CVS Health is seeking a Case Management Coordinator to support members in the Champaign area. Hybrid/remote-capable role focusing on scheduling, benefits access and education through care management tools.

Responsibilities include evaluating needs, coordinating care plans, and guiding members toward optimal outcomes while collaborating with clinicians and programs. Strong communication, analytical, and independent work skills are essential.

Qualifications

• Case management and discharge planning experience preferred
• 2 years experience in behavioral health, social services or appropriate related field equivalent to program focus
• Managed Care experience preferred
• Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
• Ability to travel within a designated geographic area for in-person case management activities as directed by Leadership and/or as business needs arise
• Excellent analytical and problem-solving skills
• Effective communications, organizational, and interpersonal skills
• Ability to work independently
• Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications.
• Efficient and Effective computer skills including navigating multiple systems and keyboarding

Responsibilities

• Evaluate members and review data to determine needs and plan case resolution.
• Identify high risk factors and coordinate appropriate referrals.
• Coordinate and monitor assigned care plan activities.
• Collaborate with clinicians to overcome barriers and support goals.
• Escalate quality of care issues through established channels.
• Negotiate options to meet member benefits and healthcare needs.
• Engage with members to promote healthy lifestyle changes.
• Participate in team meetings and mentor peers.
• Assist members in provider decision-making and shared decision processes.
• Document care and quality processes per policy and regulatory guidelines
