Qualifications

• Must reside in Kane, LaSalle or surrounding counties.
• Travel up to 50% of the time in Kane, LaSalle and surrounding counties.
• Active and unrestricted RN license in Illinois.
• 3+ years clinical practical experience.
• Independent thinker with ability to collaborate virtually.
• Excellent analytical and problem-solving skills.
• Effective communication, organizational and interpersonal skills.
• Proficient with Microsoft Word, Excel, Outlook and PowerPoint.
• Efficient and capable with navigating multiple systems.
• 2+ years case management, discharge planning and/or home health care coordination experience
• Certified Case Manager
