Qualifications

• Bachelor's degree or non-licensed master level clinician in behavioral health or human services required.
• 2 years of experience in behavioral health, social services or related field.
• Case management and discharge planning experience preferred.
• Ability to travel within a designated geographic area for in-person activities.
• Excellent analytical and problem-solving skills.
• Strong communication, organizational and interpersonal skills.

Responsibilities

• Evaluation of members and assessment of needs and benefits.
• Identify high risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities.
• Consult with care teams to overcome barriers and achieve outcomes.
• Identify and escalate quality of care issues.
• Use negotiation to secure options and services for members.
• Use motivational interviewing to engage members in healthy choices.
• Provide coaching and support to empower independent medical decisions.
• Engage with colleagues in team meetings and peer mentoring.
• Assist members in participating with providers in decision-making.
• Monitor, evaluate and document care per policy and regulations.
