Qualifications

• 2+ years experience in behavioral health or social services
• Ability to work 8:00 AM - 5:00 PM in assigned market time zone
• 2+ years of experience with Microsoft Office Applications (Word, Excel, Outlook)
• Dedicated workspace free of interruptions
• Dependents must have separate care arrangements during work hours

Responsibilities

• Evaluate members' needs/eligibility and recommend a care approach.
• Identify high risk factors and coordinate referrals to clinical case management.
• Coordinate and monitor care plan activities and progress.
• Consult with stakeholders to overcome barriers and optimize outcomes.
• Identify and escalate quality of care issues via established channels.
• Use negotiation to secure options and services for benefits/needs.
• Use motivational interviewing to boost member engagement and healthy choices.
• Provide coaching and support for independent medical/healthy lifestyle decisions.
• Assist members in engaging with providers for informed decisions.
• Document care using case management and quality management processes.
