OHARA, LLC is seeking a Field Based Nurse Case Manager (RN) to coordinate and facilitate case management for injured workers in the Des Moines, IA area. This full-time role combines field-based visits with telephonic case management to promote timely return-to-work and cost-effective care.

You will work with injured workers, employer reps, insurance adjusters, and providers, traveling regionally as needed and residing in Des Moines to support local assignments.

Formación

• Must have a current RN license.
• Associate degree and 2 years of clinical practice experience are required.
• CCM designation or applicable certification is preferred; willingness to obtain CCM designation within 3 years of employment.
• Excellent verbal and written communication skills.
• Must be able to be on call approximately 3 weeks out of the year.
• Must be proficient with Microsoft Word, Excel, and Outlook.

Responsabilidades

• Coordinate and facilitate case management services for injured workers.
• Attend appointments with injured workers and providers to support return-to-work.
• Collaborate with workers, employer representatives, insurance adjusters, and medical providers.
• Perform telephonic case management in addition to fieldwork.
