Job summary

OHARA, LLC has an opening for a full-time Field Based Nurse Case Manager in the Milwaukee, WI area, combining field visits with telephonic coordination to support injured workers toward safe return to work.

The ideal candidate is an RN with an associate degree and 2 years of clinical practice, with preferred experience in occupational medicine or neuro/ortho. CCM designation is preferred; willingness to obtain CCM within 3 years is required.

Qualifications

• Must have RN license.
• Associate degree in Nursing and 2 years of clinical practice experience.
• Experience in occupational medicine, orthopedic or neurology preferred.
• Excellent verbal and written communication skills.
• CCM designation preferred; willingness to obtain CCM within 3 years of employment.

Responsibilities

• Coordinate and facilitate field-based and telephonic case management for injured workers.
• Attend appointments with workers and providers to support safe return-to-work plans.
