Job summary

Cognizant is seeking an Emergency Department Coder to join the Health Information Management team. You will review documentation and assign ICD-10-CM, CPT, and HCPCS codes for outpatient, observation, ED, and diagnostic encounters, collaborating with physicians for clarifications in a fully remote setting.

The role requires CPC or CCS certification, with a strong track record in coding accuracy (≥98%) and adherence to regulatory guidelines.

Qualifications

• Certified Professional Coder (CPC) or CCS certification required.
• Experience coding and auditing outpatient, observation, ED and diagnostic encounters.
• Strong understanding of ICD-10-CM, CPT, HCPCS and regulatory guidelines.
• Experience with electronic encoder applications and coding tools.
• Ability to interpret clinical documentation and communicate coding queries.

Responsibilities

• Review clinical documentation and assign diagnosis and procedure codes for outpatient settings.
• Query physicians and staff to obtain clarifications on diagnoses and procedures.
• Use encoder applications and coding tools to assign accurate codes per guidelines.
• Perform peer reviews and quality audits to ensure accuracy and compliance.
• Prepare coding audit reports with findings and recommendations for leadership.
• Maintain a minimum coding accuracy rate of 98% while meeting productivity standards.
