Job summary

Cognizant seeks an Emergency Department Coder to ensure accurate, compliant medical coding across outpatient settings. You will work within Health Information Management, collaborating with clinicians and operations to support revenue integrity.

This remote role requires CPC/CCS certification and strong knowledge of ICD-10-CM, CPT, HCPCS, and encoder tools, with a proven track record of high accuracy and productivity.

Qualifications

• Certified Professional Coder (CPC), CCS, or equivalent.
• Experience coding and auditing outpatient, observation, ED, and outpatient encounters.
• Strong knowledge of ICD-10-CM, CPT, HCPCS, and regulatory guidelines.
• Experience using electronic encoder applications and systems.
• Ability to clarify diagnoses with physicians.
• Proven ability to meet productivity goals with 98% accuracy.

Responsibilities

• Review clinical documentation and assign diagnosis and procedure codes for outpatient surgery, observation, ED, and outpatient encounters.
• Communicate with physicians and staff to clarify diagnoses and procedures via coding queries.
• Use encoder applications to assign codes per policies and guidelines.
• Perform peer reviews and quality audits to ensure accuracy and compliance.
• Prepare coding audit reports and recommendations for leadership.
• Maintain coding accuracy at least 98% while meeting productivity standards.
