Job summary

Cognizant is seeking an Emergency Department Coder responsible for accurate ICD-10-CM, CPT, and HCPCS coding across outpatient and ED encounters. You will collaborate with physicians, HIM staff, and operations to support regulatory compliance and revenue cycle excellence.

You will review documentation, perform coding audits, query clinicians for clarifications, and deliver audit findings to leadership while maintaining a minimum 98% accuracy and meeting productivity standards.

Qualifications

• Certified Professional Coder (CPC) or CCS is required.
• Experience coding and auditing outpatient, observation, ED and outpatient diagnostic encounters.
• Strong understanding of ICD-10-CM, CPT, HCPCS guidelines.
• Ability to communicate with physicians about coding queries and achieve 98% accuracy.

Responsibilities

• Review clinical documentation and assign diagnosis and procedure codes for ED and outpatient encounters.
• Query physicians and staff to obtain clarifications on diagnoses and procedures.
• Use encoder tools to assign codes per policies and regulations.
• Perform coding audits to ensure accuracy, consistency and compliance.
• Prepare audit reports and recommendations for leadership.
