Job summary

CenterWell Home Health is seeking an E-Referral Specialist to support the referral process and ensure timely data collection, compliance, and follow-up on initial referrals. This role reports to the E-Referral Supervisor and serves as the primary contact for clients and referral sources.

The position is part-time or remote with required availability on weekends and selected holidays, and occasional travel to office locations for training may be required.

Qualifications

• High School Diploma or equivalent.
• Typing 40 WPM or faster.
• Ability to analyze data and generate reports using basic math principles.
• Proficiency with Word, Excel, Outlook, and Teams.

Responsibilities

• Monitor multiple e-referral systems for new referrals and alerts.
• Process emails requesting referrals from all systems and manage communications.
• Scan documents to determine patient eligibility for services.
• Track referrals and ROC decisions; provide weekly status reports.
• Communicate with branch, sales, and leadership for updates.
• Maintain ERST log of referrals and decisions.
• Maintain productivity per standards and deadlines.
• Follow regulatory policies and company procedures.
