CenterWell Home Health is seeking an E-Referral Specialist to join our remote team in the United States. You will be the first contact for referrals, handling data collection and documentation to promote continuity of care.

The part-time role is 29 hours per week (Fri 8am-5pm EST, Sat-Sun 7am-5:30pm EST) with holiday coverage as needed. Strong MS Office skills, attention to detail, and timely communication are essential.

Qualifikationen

• High School Diploma or its equivalent is required.
• Proficiency with Microsoft Office including Word, Excel, Outlook, and Teams.
• Experience with eReferral portals or EPIC systems is preferred.
• Attention to detail and ability to work with multiple systems.

Aufgaben

• Monitor multiple e-referral systems to identify new referrals and manage alerts.
• Monitor email for referral requests and perform follow-up communications.
• Scan documentation to determine patient eligibility for services.
• Monitor status of referrals and report weekly on status.
• Communicate with branch, sales, and leadership for status updates.
• Maintain ERST log with referrals and ROC decisions.
• Maintain productivity levels per departmental standards.
• Comply with regulatory policies and procedures.
