CenterWell Home Health is seeking an E-Referral Specialist to manage initial referral processes, data collection, compliance, and follow-up, reporting to the E-Referral Supervisor. This role is a part-time position with specific availability and travel expectations.

Responsibilities include monitoring multiple e-referral systems, handling referrals, assessing eligibility, communicating with branches and leadership, and maintaining the ERST log while meeting productivity standards.

Qualifications

Responsibilities

• Monitor multiple e-referral systems to identify new referrals and manage alerts and notifications.
• Monitor email for requests to pull referrals; perform follow-up and manage communications.
• Review documentation to determine patient eligibility for services.
• Track referral status and provide weekly status reports.
• Communicate with branch, sales, leadership for status updates and decisions.
• Maintain ERST log and ROC decisions daily; ensure productivity standards.
• Adhere to policies, compliance, and HIPAA requirements.
