Job summary

Adoration Health is hiring an E-Referral Specialist to handle referrals and patient information for home health services. You will respond to requests from community, physicians, patients and families, schedule admissions, and prepare paperwork for admission, acting as a liaison among patients, families, referral sources and staff.

This remote position may require occasional travel within the assigned territory to support referrals and collaboration with local branches.

Qualifications

• Associate degree preferred; High School Diploma required.
• 1-2 years of experience in a healthcare-related role.
• Proficient with MS Office (Word, Teams, Excel, Outlook).
• EMR portal experience preferred.
• Strong interpersonal communication and presentation skills.
• Ability to travel within territory as needed.

Responsibilities

• Perform required data entry accurately as per department timing.
• Manage patient information and forms and forward per procedure.
• Monitor portals for new referrals, messages, bookings, and updates.
• Coordinate with intake and branches to minimize delays in care.
