CenterWell Home Health is seeking an E-Referral Specialist to serve as the initial contact point for referrals, promoting continuity of care and ensuring accurate data collection, documentation, and follow-up on the referral process.

This part-time remote role requires strong typing, MS Office proficiency, English fluency, and the ability to manage multiple systems. You will collaborate with branches, sales, and leadership to move referrals forward and meet deadlines.

Qualifications

• High School Diploma or equivalent.
• Type at least 40 WPM with accuracy.
• Proficient in MS Office (Word, Excel, Outlook, Teams).
• Fluent in English with strong written and verbal communication.
• Good data entry and analytical skills.

Responsibilities

• Monitor multiple e-referral systems to identify new referrals and alerts.
• Check emails for referral pull requests and handle follow-up.
• Scan docs to verify patient eligibility for services.
• Track referral status and follow up through the process.
• Communicate status with branch, sales, and leadership by email/phone.
• Maintain ERST log with referrals and ROCs decisions.
• Maintain productivity per workflow standards.
• Follow regulatory policies and company procedures.
