Qualifications

• Doctor of Pharmacy (PharmD) with active, unrestricted license; multi-state a plus.

Responsibilities

• Review formulary medications and validate dosing, sig, and quantity for platform integrity.
• Provide evidence-based recommendations to CMO and operations on formulary changes.
• Ensure compliance with state and federal regs, including DEA and HIPAA standards.
• Educate Patient Care Team and clinicians on medication use, side effects, and best practices.
• Identify latest medicine trends and opportunities to present to physicians.
