Job summary

PharmcareUSA is seeking a full-time consultant pharmacist to manage clinical medication therapy for long-term care residents, focusing on optimizing regimens and improving quality of life. This role offers a mix of remote and on-site work and opportunities to educate patients and care teams.

You will be licensed in the relevant state(s) and stay current with continuing education requirements, while traveling to facilities as needed for on-site days.

Qualifications

• Licensed by the state(s) in which they practice.
• Knowledge of the legal responsibilities of the consultant pharmacist to nursing facilities and residents, including state & federal laws.
• Maintain minimum required continuing education hours established by state board of pharmacy.
• Mix of remote and on-site work.
• Travel to and from facilities within your state is required for on-site days.
• Flexible schedule with ability to help determine your schedule.

Responsibilities

• Provide clinical medication management for long-term care residents.
• Educate staff and residents on medication usage and safety.
• Collaborate with nursing facilities, hospice services and other care teams.
