Qualifications

• Bachelor's or PharmD in Pharmacy from an accredited school.
• Current pharmacist license required; dual license preferred.
• Immunization certification and IV therapy experience are preferred.

Responsibilities

• Evaluate physician orders for drug appropriateness, dosing and interactions.
• Review resident regimens monthly and report findings with recommendations.
• Document communications with physicians, nurses, and residents.
• Maintain knowledge of long-term care pharmacy laws and regulations.
• Collaborate with care teams to optimize medication therapy.
• Understand formulary management and regulatory requirements.
