Job summary

Moda Health is seeking a qualified Case Manager to coordinate and monitor care for members within an evidence-based framework. You will engage families using motivational interviewing and collaborate with multidisciplinary teams to ensure timely, appropriate treatment.

The role requires an Oregon RN or LCSW license, CCMC certification within 24 months, and strong communication skills. This is a full-time remote position with standard benefits and potential travel for meetings.

Qualifications

• Certification in case management (CCMC) required or able to obtain within 24 months of hire.
• Current unrestricted Oregon RN or LCSW license is required.
• Recent healthcare experience in acute care and/or skilled nursing care.
• Proficient with Microsoft Office and typing at least 35 wpm.
• Strong analytical, problem-solving, memory retention, organizational, and detailed orientation.
• Excellent verbal and interpersonal communication, including handling difficult customers.
• Able to work well under pressure with a professional image on calls and in person.
• Punctuality and reliable attendance are essential.

Responsibilities

• Deliver core case management activities including assessment, planning, implementation, coordination, monitoring, and evaluation.
• Conduct biopsychosocial assessments to identify needs and intervention opportunities.
• Develop individualized care plans with measurable goals to support engagement and outcomes.
• Coordinate services across behavioral health, medical, and social supports for integrated care.
• Monitor progress and modify care plans as clinically appropriate.
• Maintain documentation in line with accreditation standards and regulations.
