Lyra Health, Inc. seeks a Clinical Specialist for Complex Care to assess needs and connect clients to appropriate care by phone and email. Remote, full-time role with Monday–Friday hours, focusing on high-risk behavioral health cases.

The position requires an MSW or equivalent, active licensure (LCSW preferred), and strong crisis-management skills. Compensation includes comprehensive benefits and a tech-friendly work environment.

Qualifications

• Master’s degree in clinical social work or equivalent; LCSW or equivalent independent licensure required.
• Experience managing high-risk clinical situations and navigating health plans.
• Ability to work independently in a remote environment and within a small team.

Responsabilités

• Independently manage a caseload of complex, multi-need clients with long-term case management.
• Support clients through levels of care using Motivational Interviewing and engagement strategies.
• Respond to crises with brief risk assessments and safety planning to connect to treatment.
• Communicate with clients and providers via email, phone, and text as needed.
• Identify care options using proprietary matching software and facility resources.
