Job summary

Alignment Health is seeking a Complex Care Registered Nurse to join our remote care pod focused on high-acuity Medicare Advantage members. You will own the member journey, coordinate care across disciplines, and ensure proactive, timely interventions in a fully virtual delivery model.

Qualifications

• 3+ years RN experience in complex care, care management, or transitions of care.
• Experience with Medicare Advantage, managed care, or value-based care.
• Experience in telehealth/virtual care delivery and remote member engagement.
• Familiarity with HEDIS, care gap management, and care coordination workflows.

Responsibilities

• Own the member journey and care coordination for high-acuity Medicare Advantage members.Build relationships with members and caregivers via telehealth outreach.Own transitions of care and post-discharge follow-up to reduce readmissions.
• Perform medication reconciliations and monitor clinical indicators across the pod.
• Escalate clinical concerns and support APC and RMO decision-making.
• Coordinate cross-disciplinary care with PCPs, APCs, Health Coaches, Care Coordinators, and Social Workers.
• Support HEDIS and quality measure initiatives through timely follow-up and education.
