• Coordinate outreach resources for members and promote awareness of care/services.
• Assist members with community and care resource inquiries and opportunities.
• Support member-related correspondence and educational materials for care facilitation.
• Document and maintain all community resources to meet health plan requirements.
• Provide assistance to the clinical team, including outreach, education, and informal guidance.
