Making Space in the United Kingdom is hiring a Community Navigator to connect adults in Cambridgeshire with local services, groups and emotional support to reduce isolation and promote independent living.

The role is predominantly home-based with some local travel. Candidates should have an NVQ level 2 in Health and Social Care (or be willing to work toward it) and a track record of assessing needs and creating outcome-focused plans.

Qualifications

• NVQ Level 2 in Health and Social Care or willingness to work towards this.
• Experience assessing needs and producing outcome-focused plans is essential.

Responsibilities

• Link adults with local services, community groups and emotional support.
• Provide information, guidance and signposting to help clients explore options.
• Support positive mental health and independent living through tailored information and referrals.
