Job summary

The Columbus Organization is seeking a Community Navigator to evaluate and refer clients, conduct site visits, address service gaps, and support crisis intervention for the IDD population. You will collaborate with stakeholders statewide to ensure consistent, high-quality care.

Responsibilities include assessments, care planning, documentation, and outreach, with travel within Sussex North County, Delaware. Must reside in Sussex County and have reliable internet and transportation.

Qualifications

• Minimum one year of experience in health or human services, interviewing and assessing needs.
• Associate's degree OR certificate OR at least one year of relevant interviewing/assessment experience.
• Experience contributing to client service plans and making informed recommendations.
• Knowledge of applicable laws, regulations, policies, and procedures.
• Proficiency with electronic information systems for data entry and reporting.
• Strong written communication in English and Spanish.

Responsibilities

• Serve as primary liaison for participants, families, providers, and partners to coordinate services.
• Conduct assessments and develop person-centered service plans aligned with goals.
• Monitor and evaluate service delivery; address gaps and ensure safety and satisfaction.
• Provide casework, resource education, and crisis support when needed.
• Maintain HIPAA-compliant electronic records and documentation.
• Build natural support networks and promote engagement with resources.
• Outreach to stakeholders to expand awareness and access to services.
