Job summary

Open Sky Community Services is hiring a Community Supports Navigator in Massachusetts to provide case management and outreach to individuals with mental health or co-occurring disorders at high risk of admission to 24-hour treatment settings.

You will develop support plans, complete required assessments, and coordinate care with community agencies, healthcare providers, and families. The role includes on-call rotations and transportation assistance to essential appointments.

Qualifications

• High School Diploma or GED required.
• Valid Driver's License and reliable insured transportation required.
• Minimum 2 years' experience working with or lived experience with homelessness, justice involvement, adults with serious mental illness, substance use disorders, or underserved populations.

Responsibilities

• Develop a Support Plan with the member according to timelines and specifications.
• Complete Assessments and Support Plans as prescribed.
• Act as liaison between members, families, and community agencies to coordinate treatment.
• Collaborate with medical/behavioral health providers to obtain active care plans.
• Provide temporary transportation assistance to medical/behavioral health appointments during transition to community resources.
• Participate in On-Call rotations.
