Job summary

Open Sky Community Services in Massachusetts is hiring a Community Services Navigator to provide case management and direct outreach within our Community Supports Program.

You will develop Support Plans, coordinate with community and clinical partners, assist with transportation during transitions, and participate in on-call rotations to support individuals at high risk for admission to 24-hour treatment settings.

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License with acceptable driving history and reliable, insured transportation.
• Minimum of least 2 years’ experience working with or lived experience with homelessness, justice involvement, adults with serious and persistent mental illness, substance use disorders, and underserved populations, required.
• Excellent communication, documentation, time management and organizational skills required.

Responsibilities

• The Community Services Navigator will provide case management and direct outreach support to assigned caseloads, including members enrolled in Community Services Programs, Recovery Support Navigation, Behavioral Health Justice Involved and/or Community Support Program for People Experiencing Chronic Homelessness.
• Develop a Support Plan with the member that guides services in a way that is consistent and supportive of other existing comprehensive care plans. Complete all Assessments and Support Plans according to prescribed timelines and specifications.
• Act as a liaison between members, community agencies, and families to coordinate treatment with employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others.
• Provide temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources.
• Participate in an On-Call rotation.
