Optima Medical Management Group is seeking a Community Health Worker (CHW) to serve as a liaison between community members, healthcare providers, and social services, primarily remote within our care pod. The CHW will conduct outreach, provide education on health topics, and connect members with medical and community resources.

Qualifications

• High school diploma or GED required and CHW certification.
• 1 year minimum experience in case management, member care, customer service, call center, or related roles.
• Strong communication, cultural sensitivity, and ability to work with diverse populations.

• Conduct in-person visits and provide health education to community members.
• Assist individuals in accessing healthcare services and community resources.
• Assess social determinants of health and identify barriers to care.
