Job summary

Optima Medical Management Group seeks a Community Health Worker (CHW) to serve as a trusted liaison among community members, healthcare providers, and social services. The role operates mainly remotely within a multidisciplinary Care Team, focusing on outreach, health education, and connecting members to medical and community resources.

Under supervision of ECM Leadership, you will work with Lead Care Managers, MAs, and RNs to improve health outcomes, reduce barriers to care, and promote

Qualifications

• High school diploma or GED required.
• Minimum 1 year experience in case management or member care.
• CHW certification required.
• Valid California driver’s license and vehicle insurance required.

Responsibilities

• Conduct in-person visits to community members and provide health education.
• Assist individuals in accessing healthcare services and community resources.
• Assess social determinants of health and identify barriers to care.
• Support care coordination with providers, social workers, and agencies.
• Document interactions and referrals accurately in designated systems.
• Advocate for members and help resolve barriers to access.
