Job summary

Point32Health is seeking a Community Health Worker to assist members in managing health goals in the community. The CHW collaborates with the Integrate Care Management team to improve knowledge, motivation, and adherence to care plans.

The role includes coordinating safety net services, education, housing, healthcare access, and support for social determinants of health. Travel to member homes and community sites is expected as needed.

Qualifications

• Excellent written and verbal communication skills.
• Working knowledge of Microsoft Office applications.
• Ability to utilize an Electronic Medical Record.
• Bilingual or multilingual.
• Knowledge of welfare reform, homelessness, and community resources.

Responsibilities

• Perform regular SDoH assessments via telephonic and in-person visits.
• Provide health education and outreach in community settings.
• Bridge the gap between communities and health/human services.
• Advise on health-related barriers and assist with benefits navigation.
