Job summary

CVS Health Corporation in Louisiana is seeking a Community Health Worker (CHW) for a full-time remote role. The CHW will serve as a bridge between members and the healthcare system, conducting home visits, outreach, education, and care coordination under supervision.

Qualifications

• Must reside in LA and within the assigned region/parishes: Region 1 (Orleans, Jefferson, St. Bernard, Plaquemines).
• If not CHW certified at time of hire, must complete a Certified Community Health Worker /Promotora certification program as recognized by the State of Louisiana within 12 months of start date.
• Ability to travel 60-75% of the time. Must have access to reliable transportation.
• 1-2 years' experience providing clinical support services.
• 1 year of community outreach experience and knowledge of LA community resources
• 1 year' experience working with clients with clinical needs (physical and/or behavioral health)
• Proficiency with MS Office Suite and electronic health records (MS Teams, Outlook, Word, Excel)
• Flexibility to work outside core hours and up to 2 evenings per week
• 2 days weekly until 9pm local

Responsibilities

• Conduct home visits to reinforce care plans and adapt to member culture and language.
• Educate providers on social determinants of health impacting care.
• Build strong relationships with members, providers, and staff.
• Help reduce barriers between clients and institutions.
• Assist with care plan implementation and education during in-home visits.
• Assist members in identifying socio-economic issues affecting health.
• Identify gaps to strengthen care systems and connect to community resources.
• Support medication adherence by reviewing lists and obtaining refills.
• Teach disease self-management and health education.
• Accompany members to appointments as needed.
• Enter and maintain records in EHR and compile program reports.
• Travel extensively to client homes and outreach sites.
