Job summary

Optima Medical Management Group is seeking a Community Health Worker to join the care team, mostly remote, serving diverse populations across California.

As a trusted liaison, you will conduct outreach, educate members on health topics, and connect them with medical and social resources. Requires CHW certification, a high school diploma, and a California driver's license. Pay range is $25–$28 per hour, with full-time benefits.

Qualifications

• High school diploma or GED required.
• Minimum 1 year experience in case management or member care.
• CHW certification required.
• Valid California driver’s license and vehicle insurance required.
• Strong communication and cultural sensitivity.

Responsibilities

• Conduct in-person visits and educate on health topics.
• Assist members in accessing healthcare and social services.
• Assess social determinants of health and barriers to care.
• Support care coordination with providers, care managers, social workers, and community agencies.
• Travel to community sites and document interactions in systems.
• Maintain member confidentiality and comply with regulations.
• Participate in outreach events and member huddles.
• Report to the office for meetings as needed.
