Job summary

Mercy is seeking a Community Health Worker I to serve as a trusted liaison between patients, healthcare providers, and community resources in a virtual outpatient setting. You will educate patients on healthcare coverage options, guide them through access to care, and connect them with community services to improve health outcomes.

You will manage a caseload, conduct outreach, document encounters in the EMR, and participate in ongoing CHW training to stay current with best practices.

Qualifications

• Education: High School Diploma or equivalent required.
• Experience: One year of Community Health Worker experience or equivalent.
• Certification: Mercy-provided Community Health Worker Training Program within 6-12 months of hire.
• Knowledge, Skills & Abilities: Strong interpersonal, relationship-building, collaboration, and communication skills; ability to work independently; compassion and commitment to serving diverse populations.

Responsibilities

• Advocate for patients experiencing health-related social needs and guide them through healthcare and social service systems.
• Coordinate connections to community resources and build relationships with service providers.
• Document patient interactions and maintain records in the EMR; support program reporting.
• Manage a caseload and conduct outreach via calls, messaging, and approved platforms.
• Pursue Community Health Worker training and certifications; stay informed on community resources and best practices.
• Perform other duties as assigned to support department goals.
