Optima Medical Management Group is seeking a Community Health Worker (CHW) to serve as a trusted liaison between community members, healthcare providers, and social services. The role is primarily remote, collaborating with Care Team members to improve health outcomes and reduce barriers to care.

The CHW will conduct outreach, provide health education, and help navigate medical and social services, with travel as needed.

Qualifications

• High school diploma or GED required.
• Minimum of 1 year experience in case management, member care, customer service, call center, or member care.
• CHW certification required.
• Valid California driver’s license and valid vehicle insurance required.

Responsabilités

• Conduct in-person visits to community members and provide health education.
• Assist individuals in accessing healthcare services, social services, and community resources.
• Assess social determinants of health and identify barriers to care.
• Support care coordination with care teams, social workers, and community agencies.
• Remote member engagement with Lead Care Managers, MAs, and RNs.
• Accompany members to appointments and assist with enrollment navigation.
• Maintain contact via calls, home visits, and telehealth platforms.
