Job summary

Community Health & Wellness Center (CHWC) is seeking a Community Health Worker to assist patients and their families in accessing community resources and adopting healthy behaviors. This role involves close collaboration with health providers and the management team, ensuring patients receive comprehensive care through a supportive approach.

The ideal candidate will facilitate outreach, conduct home visits, and provide education, effectively bridging the gap between vulnerable populations and healthcare services in Connecticut.

Qualifications

• 3 years of health and/or social service experience.
• Ability to work independently and as part of a team.
• Must speak, write and read English proficiently; Spanish preferred.

Responsibilities

• Help patients navigate healthcare and social service systems.
• Manage assigned caseload of patients.
• Create connections between vulnerable populations and healthcare providers.
