Job summary

Wellcentric DC in Washington, DC is seeking a Community Health Worker to serve as a bridge between enrollees, the healthcare system, and community resources. The CHW will conduct outreach, perform home visits within scope of practice, and help reduce barriers to care.

Collaborating with the Case Management team, the CHW will educate, advocate, and assist members in navigating services, coordinating appointments, and following through with care plans to improve engagement and outcomes.

Qualifications

• High School Diploma or GED is required.
• Experience with community organizations, validated by references.
• Driver’s license with a clean driving record.
• Ability to read, write, and speak English.
• Commitment to compliance with federal, state, and regulatory laws.
• Ethical behavior and integrity.
• Professionalism and excellent customer service.
• Ability to work with people from diverse backgrounds.

Responsibilities

• Outreach to enrollees by phone and in home/community visits.
• Build trusting, culturally responsive relationships.
• Provide health education and advocacy.
• Collaborate with Case Managers on care management.
• Help navigate primary care, behavioral health, and social services.
• Schedule appointments and coordinate transportation.
• Document activities in the electronic medical record.
• Participate in trainings and team meetings.
