InterCommunity Health Care is seeking a Community Health Navigator to support Hartford residents in accessing medical, behavioral health, and social services. This role blends outreach, patient navigation, health education, and barrier-reduction strategies to promote whole-person health.

The CHN conducts outreach to identify individuals not connected to care and reengages existing patients, using SDOH data to create individualized navigation plans and provide hands-on support to improve health

Qualifications

• Must have strong communication with diverse populations.
• Must maintain accurate documentation and manage multiple priorities.
• Bilingual Spanish/English preferred; driving required.

Responsabilités

• Conduct outreach to identify individuals not connected to medical home and engage patients who missed appointments.
• Complete SDOH screenings and document findings; develop individualized navigation plans.
• Coordinate medical, behavioral health, and social service appointments and provide transportation support.
• Assist clients with care plans, MyChart access, telehealth navigation, and understanding health systems.
• Maintain up-to-date documentation of assessments, care plans, progress notes, and services rendered.
